COMorbid disease MANagement Database
A free chronic disease patient registry is available allowing the entering and tracking of any disease, test/laboratory result, examination, immunization, patient education, medication, assessments, general counts and other self defined items over time.
The program has 75 items already loaded, including all elements needed for the National Quality Indicator report and American Cancer Screening Guidelines. Registry users can add their own. They are automatically added to input screens and all reports. If the items are tests or exams, they are automatically added to the graph reports. All items can be flagged as gender specific to reflect correct denominators, and tests can have range values for both male and female to report who is out of range.
Twenty-one reminder rules are in the system, including all the CMS Physician Office indicators and a complete set of the American Cancer Society cancer screening guidelines. Users can add their own. These reminders can be triggered by any combination of disease state, age, gender and days until due.
Included is a drug alert module to allow identification of inappropriate drug use, drug-to-drug interactions, and drug-to-disease interactions.
The program allows users to export all data, the last-report data, only last-visit data and confidentially safe data. Data in COMMAND format can also be exported, so users can send to other COMMAND users. They in turn can import the data. Patient demographic information, profile data, and visit data can also be imported using easy ready-to-use templates. Sample data demonstrate these functions.
Whenever possible, data can be entered using defaults and single clicks to minimize typing. It runs on MS Access 2000 or 2003, but a license free version of MS Access 2000 Runtime is included so no purchase is required. System requirements are minimal.
