IQH Privacy Policy

Information & Quality Healthcare (IQH) is committed to protecting the privacy and security of our Web site.  To protect the privacy of visitors, we provide this notice explaining our online information practices and the choice you make about the way your information is collected and used. 
 
As our privacy policies change in significant ways, we will make every effort to notify you of the changes.  Minor changes to the policy may occur that will not affect our use of individually identifiable information.  When the privacy policies change in a way that significantly affects the way we handle personal information, we will not use the information we have previously gathered without obtaining consent from the appropriate individual.  We will post privacy policy changes on our Web site in a timely manner.

Personal Information

We may collect personally identifiable information only if specifically and knowingly provided by you.  Any personally identifiable information requested by IQH, i.e., contact name, address, phone or fax number, or email address, is protected by our confidentiality policy. This information is used solely by IQH for the information or service you requested. IQH will protect the personal information that you share with us.  IQH does not disclose, give, sell, or transfer any personal information to third parties. 

Electronic mail is not secure against interception.  If your communication is confidential or sensitive, please send it by U.S. mail instead.  Do not send confidential information, Social Security numbers, Medicare numbers, etc., to IQH via email.
 

Web Site Policies

IQH’s Web site automatically collects and records the following information about each visitor:  date and time of visit, pages visited on our site, search engine and key words used to find our site, type of computer and browser the visitor is using, and the location of the visitor’s machine by country, not street address.  We use this information to improve our service to you and do not share it with third parties.

 

CME Privacy and Confidentiality Policy

Information & Quality Healthcare (IQH) respects and values your privacy and the confidentiality of the information you choose to share with us. We are committed to protection of this information and will assure your privacy when participating in CME activities on our Website.

For questions regarding our Internet privacy policy, please contact the Human Resources Manager/Compliance Officer.

When you enroll in a IQH hosted course offering, personal information collected is for the purpose of processing your registration and awarding CME credit. IQH serves as hosting agent for CME testing and will provide the information collected to the organization that awards the CME credit.

Authorized users may download materials from this site if any reproduction of the material is for noncommercial purposes; if it does not alter the material; and if appropriate credit is given.
